Client Information Sheet (domestic case) - Please complete this form prior to coming to our office for a consultation.  Please complete as much of the information as possible, as all of the facts contained on this form are applicable in divorce and/or custody cases.  Please also complete the Affidavit of Financial Condition, below.

Affidavit of Financial Condition - This form is REQUIRED by the court for any case in which financial relief is being requested by either party.  In general, we also request this form from all new clients at the outset of our representation as it is imperative that we have a complete understanding of the assets in every divorce case and of the income in every custody case.
